Position Title: Advocacy, Planning, and Resource Diversification Consultant

 Background

The USAID Tuberculosis Private Sector (TBPS) project supports private sector health providers to increase case detection, enhance access to state-of-the-art diagnostics and medicines, increase TB case notification from the private sector and promote public-sector stewardship for sustained response and commitment to achieve a Tuberculosis (TB)-free Indonesia. The overarching goal of TBPS is to increase the number of care-seeking clients in the private sector who are correctly diagnosed and notified and successfully complete TB treatment. In late 2020, TBPS conducted a stewardship capacity assessment designed to understand overall stewardship capacity for the Tuberculosis (TB) program with respect to coordination/implementation, multi-sectoral engagement, and sustainable funding in two TBPS target districts: Medan City with an existing District Action Plan (DAP) (2017–2021) and Gresik Regency without an existing DAP. The key findings and recommended next steps are documented in the stewardship report.

The goal of the consultancy will be to support districts in specific governance, advocacy, and planning areas based on the findings of stewardship capacity assessment and to document key lessons learned that will be relevant to other districts interested in strengthening stewardship of their overall TB programs. These activities will be conducted in close collaboration with district officials and other district-level stakeholders and in collaboration with TBPS District Technical Officers (DTOs) and technical team with technical guidance and support from Meridian’s Senior Manager, Ms. Cindi Cisek.


Scope of Work

Support districts in developing an internal and external advocacy strategy. Within both Medan City and Gresik Regency, the consultant’s role will be to support District Health Office/DHO’s (Infectious Disease Prevention and Control Section) in ensuring that internal advocacy and support for the TB program is strengthened. The consultant will facilitate support for both districts, liaising with the Head of Infectious Diseases Prevention and Control Section, to develop a TB advocacy strategy for internal and external stakeholders. In the case of Medan, this will include plans to evaluate the existing TB DAP and begin planning for follow-on DAP. In the case of Gresik Regency, this will include supporting the district in ensuring broader non-governmental coordination. In both districts, this will include identifying specific government and non-governmental actors based on their political power and ability to negotiate allocation of resources, including incorporating TB program activities across different budget line items. Local parliament members and Civil Society Organizations (CSOs) should be involved in monitoring so that they are aware of and understand district TB programs. At the national level, consultant will present the stewardship capacity assessment results to National Tuberculosis Program/NTP (conducted in Q1 2021) and gather inputs from NTP to support the development of district advocacy strategies.

Deliverables: 1) Presentation of the stewardship capacity assessment to NTP, 2) Notes from initial sessions with DHO/Infectious Disease Prevention and Control Section, 3) Facilitation of district-level meetings with key stakeholders, 4) Development of a brief advocacy strategy for each district (no more than 5 pages) with specific actions/audiences to be included. 5) Notes from Facilitation of national-level meetings with NTP.

 Strengthen cross-sectoral TB planning and accountability. The consultant will support districts to outline specific steps to ensure that annual sectoral planning and budgeting include specific TB activities and indicators as presented in the TB DAP and other strategic planning documents. TB activities and indicators should also be included in the RPJMD and annual district government work plan (RKPD) to increase accountability.

This will include support to DHO in preparing to incorporate specific TB indicators in the RPJMD and annual district government work plan (RKPD) as well as the District Incentive Fund (DID), to reward achievement of specific indicators, within the Finance Ministry so that district is motivated to accelerate TB elimination.

Deliverables: 1) Mapping of process and timeline for integration of TB indicators into each phase of RPJMD, RKPD, and District Incentive Funds, including specific recommendations for Medan City and Gresik Regency. 2) Summary report (internal) of key actions taken by districts.

Support districts in diversification of TB funding alternatives. The consultant will work with district stakeholders (DHO, BAPPEDA, and BPKAD) to map the overall process for special transfer fund (DTK) and general transfer fund (DTU) under the Directorate of Finance Balancing within the Ministry of Finance—as well as the PP No. 17/2017 that regulates the planning and budgeting (Krisna application), including the development of any supporting data that is required. The consultant will identify specific critical action points for the various stakeholders, including the DHO.

Deliverables: 1) Mapping of process and timeline for multiple financing options, including specific recommendations for Medan City and Gresik Regency. 2) Summary report (internal) of key actions taken by districts.

Support development of national-level policy brief. Based on the processes identified and mapped for both Medan City and Gresik Regency, the consultant will also provide inputs to a national-level policy brief to be drafted by Meridian.

Consultancy Location and Travel

The consultancy will require travel to Medan City and Gresik Regency to meet with government officials and TBPS District Technical Officer and Field Operation Manager. Given the current COVID-19 epidemic, it is likely that some encounters will be converted to virtual format to allow physical distancing. The expenses of travel/ accommodation will be adjusted according to FHI 360 standards.

Consultant Requirements

§  Bachelor’s degree in health economics, health financing, governance, or similar field

§  At least 15 years’ professional experience in Advocacy, Governance & Health Financing

§  Familiarity with MOH’s National TB strategy through previous work experience related to the MOH is a plus

§  Familiarity with district-level budgeting and planning cycles

§  Experience working with Ministry of Finance, BAPPENAS & BAPPEDA

§  Experience in public-private dialogue, multisector collaboration, consensus building, and facilitation

§  Fluency in English and Indonesian and strong writing, communication, and reporting skills

§  Excellent interpersonal and communication skills

§  Ability to work within tight deadlines

§  Flexibility, adaptability, and resourcefulness
